  • Enhance interdisciplinary collaboration and communication between primary care providers and specialists to optimize patient care delivery.
  • Streamline and standardize referral workflows to improve care coordination and timely access to specialty services.
  • Develop shared knowledge and mutual understanding among all healthcare team members regarding initial workup, management, and treatment of common specialty conditions prior to or in lieu of referral.
  • Improve patient experience and outcomes through more efficient care transitions and appropriate utilization of specialty services.
  • Establish clear interprofessional protocols and role definitions for specialty engagement, including specific responsibilities for each team member in the pre-referral, referral, and follow-up processes.
