Project ECHO: Telehealth for Transitional Care for Adolescents and Young Adults with Developmental Disabilities

Series Objectives:

This activity is designed to improve learner competence in performing telehealth visits for adolescent and young adult patients with developmental delays and/or intellectual disabilities and to improve management of common medical concerns addressed in this patient population.

Series Target Audience: Primary Care Providers (Pediatrics, Internal Medicine, Family Medicine) and Pediatric Specialists who care for adolescents and young adults with developmental and intellectual disabilities

Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn 1.0 MOC points in the American Board of Internal Medicine's (ABIM) Maintenance of Certification (MOC) program. Participants will earn MOC points equivalent to the amount of CME credits claimed for this activity. It is the CME activity provider's responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit.
