Objectives: 

Enhance anesthesiologists' and anesthetists' awareness of adverse events and anesthesia complications occurring in our department and nationwide. Provide a forum for the discussion of complications and medical errors that enhances providers' ability to develop solutions to these problems. Enhance patient safety and improve quality of care.
