Objective(s):

1. Describe key social determinants of health and explain how they influence clinical outcomes, care access, and health equity in APP practice settings.  
2. Integrate SDOH screening and documentation into routine patient encounters to identify unmet social needs that impact diagnosis, treatment plans, and follow‑up care.  
3. Implement patient‑centered interventions by leveraging interprofessional collaboration, community resources, and referral pathways to address identified SDOH barriers.
