Topic: South Carolina Pediatric Palliative and Hospice Care Collaborative
Objectives:
- Describe Family Connection SC's mission and the core services it provides to families (parent-to-parent support, IEP/504 and special education navigation, healthcare and insurance system navigation, transition planning, and resource connection).
- Identify appropriate scenarios for referring a family to FCSC — such as a new diagnosis, difficulty navigating IEP/504 or early intervention processes, insurance/Medicaid/TEFRA navigation, need for parent-to-parent peer support, or transition-age planning.
- Understand how to make a referral and what families can expect after being connected to FCSC.
