Series Objectives:
We have an opportunity to improve provider documentation within the EHR to accurately capture patients' complexity and severity.  The target audience is not consistently documenting accurate and complete documentation that is able to be captured in facility coding and claim submission.
 
Learners will be able to efficiently capture patients' complexity and severity by documenting patient conditions, comorbidities, and procedures appropriately.
