This is a live activity to allow for interaction between learners to discuss cases that had an unpredictable outcome – near miss, obvious opportunity for improvement, or unintended harm.  Learners from all areas of GHS are invited to participate, as well as individuals from risk management.  The goal of this activity is not to place blame for an unpredictable outcome, but to assess the outcome to improve processes to prevent the same outcome from happening again.
